Abstract:

While long-term treatment of opioid use disorder (OUD) with buprenorphine shows great promise in curtailing the opioid overdose crisis, few patients are initiated or maintained on treatment after diagnosis. The OUD care cascade provides a conceptual framework for benchmarks that reflect effective treatment initiation and maintenance. However, few models have been developed to analyze observational data appropriate for this longitudinal framework, often relying on cross-sectional evaluations alone. We used a multistate transition model to characterize individual-level transition through OUD care stages. We analyzed routinely-collected healthcare data from the Rhode Island All-Payer Claims Database (RI APCD), 2011 to 2019. We described how to transform electronic health records into well-defined stages of the OUD cascade and adapted approaches for identifying transition probabilities to this setting. We used this model to evaluate factors associated with initiation, short- and long-term retention and barriers contributing to disengagement from care. This modeling approach will aid in developing future interventions to sustain patient recovery among those with OUD in RI.
