One of the promises of the learning health care system is the opportunity to learn which interventions are effective at the same time that those interventions are provisioned. When implementing such studies, one needs to strike a balance between statistical/analytic concerns and clinical/patient concerns. The best pragmatic trials are those that can avoid interfering with provision of clinical care while still learning whether that care was effective. In this talk we discuss two different study designs to assess two different interventions. The first is a cluster randomized cross-over trial to assess a telephone intervention for outpatient no shows visits. The second is a step-wedge based design to assess care management for children with medical complexity. Both designs utilize clinical decision support tools to prioritize who ought to receive care in conjunction with randomization in the delivery of that care to be able to learn how effective it was. Being able to design and implement such studies – while using limited additional resources – is an important component of a functioning learning health care system.
