Administrative data such as ICD diagnosis codes have been frequently used for cohort identification and clinical phenotyping. In this presentation, I will use a few studies of codes usage patterns to illustrate the lack of granularity and specificity in diagnosis codes when compared to nuanced narratives in electronic health records and how such limitations impact phenotyping and cohort identification outcomes. Best practices for using such administrative data and more research on clinical terminology user behaviors are recommended.
