Title: Data-Driven Community Based Programming: a Statistical Analysis of Heart Disease Prevention Initiatives in Oklahoma City-County

The primary causes of death in Oklahoma City-County is cardiovascular (CVD) disease, as published in the most recent OKC-Co Wellness Score. Robust analysis identified the highest mortality rate zip codes across the metro. Combining the results with strategic planning and operational partners, the Oklahoma City-County Health Department expanded the CVD prevention program, My Heart, to reverse this disease burden for future generations. My Heart collects clinical information, laboratory results, and quality of life data at various points in time throughout program enrollment. Quantitative and qualitative analysis, including demographics, biometrics and social determinants, are conducted at least quarterly to monitor progress. Using SAS and R, a full evaluation was conducted for nearly 1,000 clients to identify trends in program results and evaluate adherence. Additionally, logistic regression modeled lost to follow up and multiple regression modeled biometric outcomes. Wilcoxen signed rank test indicated a significant decrease (p< 0.0001) in blood pressure with multiple improvements in quality of life and reduced CVD risk factors across cohorts, as detailed in the full publication.
