Hospitalizations account for one-third of the annual healthcare costs in the USA. A large percentage of hospital readmissions can be avoided or prevented. The University of Iowa Hospitals and Clinics (UIHC) deploys nurse-led transitional care teams (TCT) in order to prevent unnecessary hospital readmissions. TCT provides patients with disease self-management, and clear instructions regarding discharge and hospital revisit. In this study we explore treatment effect following a quasi-randomization through propensity scores matching. Inter-readmission times were analyzed and appropriate recurrent event methods were used to explore differences in inter-readmission survival.
