There is increasing political interest in harnessing the vast amount of health information maintained by health care organizations and insurers for research purposes. These electronic health records and insurance claims are captured for administrative and clinical purposes during the course of routine patient care, and they offer great promise to improve public health decisionmaking. NIH's Big Data to Knowledge, FDA's Sentinel Initiative, and PCORI's patient-centered clinical research network exemplify this growing 'health care data analytics' effort to generate meaningful medical evidence from large data sources that may not be of the quality typical for research. But bigger data don't necessarily yield better answers. How can we tell when these data are fit for research? How do we convey their limitations and boundaries? When is it reasonable for decisionmakers to act on the results? What is a statistician's role in this new learning process? This presentation will address these questions and more. Examples from ongoing comparative safety and effectiveness studies and pragmatic trials in this database setting will be used to provide context and frame the discussion.
