Abstract:

Due to high morbidity and mortality of antibiotic resistance among hospital acquired infections, regional collaborative on antibiotic stewardship program beyond individual hospitals has been underscored in recent years. To inform policy makers and stakeholders of prevention success nationwide, summary measures like standardized estimates may be useful. Under the Centers for Medicare & Medicaid's inpatient quality reporting program requirement, U.S. hospitals have begun to report central-line associated blood stream infections and antibiotic susceptibility data to CDC's hospital surveillance system since 2011. We estimated standardized %resistance of methicillin-resistant Staphylococcus aureus (MRSA) for each calendar year as a function of predicted marginal by logistic regression after controlling for differences in covariate distributions such as age and gender. These standardized results were compared like %resistance for direct comparison across the years. There was significant interaction of calendar year with gender but not with age. It is concluded that age-standardized %resistance can be presented to assess the MRSA prevention success over time, separated by gender.
