Abstract:

Penetrance distribution is a key quantity for estimating carrier probability of disease-susceptibility genes and individual risk of the cancer incidence. In Li-Fraumeni Syndrome (LFS) study, which involves multiple cancer types of interest, cancer-specific risk estimation is often desired to provide better treatment that is personalized to the patient's need. In this regard, we develop a new Bayesian approach for cancer-specific age-at-onset penetrance estimation under competing risk framework. In segregation analysis, family histories carry crucial information on genetic treats of interest and family-wise likelihood is commonly exploited to incorporate these family histories in estimation. In our work, the family-wise likelihood under the newly developed competing risk model is defined and the associated Bayesian estimation scheme is then proposed. We illustrate the new method to a cohort of 182 LFS families identified through probands with pediatric sarcoma treated at MD Anderson Cancer Center from 1944 to 1982.
