Title: Profiling a Large Number of Centers for a Rare Event

Abstract:

The statistical evaluation of center performance is becoming an increasingly important component of health care research. The application which motivated our work is characterized as follows: there are a large number of centers (over 5,000) and patients; the event of interest is recurrent; the event rate is relatively low; there exist unmeasured event rate predictors; and the objective is to estimate center effects. To carry out the analysis, we combine two existing works: first, methods to compare the observed versus expected center-specific number of events; second, computational techniques to approximate the proportional rates model with a generalized linear model. The application features the comparison of facility-specific transfusion rates among dialysis patients using national data.
