Title: How Biomarker Collection Date Influence Death Rates

Abstract:

Objective: Determine how selection bias is introduced by a researcher's choice on the collection time of a biomarker when defining annual cohorts to examine death rates.

Method: Use data from Kaiser Permanente data warehouse to examine annual death rates for Chronic Kidney Disease (CKD) when different collection times in the follow up spectrum are applied.

Result: Trend lines and magnitudes of death rates for the reporting years from 2000-2009 were found to be dependent on when the biomarker was collected during the follow up period for CKD patients. By selecting the last data point to define the reporting year of the cohort (group1), the rates did not decline as steeply as when selecting any data point observed prior to or in the reporting year (group2). Death rates for group1 are ten times higher than rates observed for group2. Death rates by year of first collection were 6 times greater for group1 compared to group2.

Conclusion: This study found that the collection time of a biomarker can influence annual death rates and may introduce selection bias. Researchers may need to consider the collection time of a biomarker when defining reporting year cohorts.
