Non-parametric approaches can be applied in clinical trials for analyzing exposure adjusted incidence. Two approaches will be discussed, one may use the mean of the ratio of number of events over follow-up time within each subjects as an estimator and the other use the ratio of the mean of the number of events over the mean follow-up time in all the subjects. Asymptotic properties of all estimators were studied through numerical simulations. Nonparametric ratio of the means is a consistent estimator and to be recommended in analyzing over dispersed exposure adjusted incidence data. When sample size is small, resampling-based approaches can help yield satisfactory results
