Abstract:

Cancer screening guidelines should be based on the cancer risk implied by each combination of screening tests. However, risk is difficult to estimate from screening data where decisions to treat are strongly based on the history of screening test results and where participants return at irregular and long intervals. Using data on over 330,000 women undergoing cervical cancer screening, we modeled time to cervical cancer or precancer jointly with longitudinal models for screening tests (human papillomavirus (HPV) test results and Pap smears) using a shared parameter approach. We suggest how risk estimates from this model might inform cervical cancer screening guidelines.
