In 2008, the Food and Drug Administration (FDA) issued an alert regarding increased risk of suicidal thoughts and behavior related to use of antiepileptic drugs (AEDs). The FDA scientific advisory committee concluded a significant positive association between AEDs and suicidality, but voted against placing a black box warning on AEDs for suicidality. To investigate the causal relationship between use of AEDs and risk of suicide attempts, we implement a propensity score matching analysis for a cohort of patients with bipolar disorder. To accommodate a person-year suicide attempt rate outcome, we modify the potential outcome framework, compare different estimation strategies and elaborate their causal interpretations. The important information on pre-treatment suicide attempts is incorporated in several ways, including matching, regression modeling and a difference-in-difference type of adjustment.
