Abstract:

In observational studies, unlike clinical trials, enrollment need not correspond to a clinically important event such as initiation of a new therapy. Estimating survival from time of enrollment may therefore be less useful than estimating survival from time of diagnosis for answering certain clinical questions. Left truncation methods make it possible to estimate survival from diagnosis, and these methods are becoming more widely used now that they have been incorporated into standard software packages such as SAS. We have applied these methods in REVEAL, a 3500 patient disease registry, to estimate survival from diagnosis. We have also extensively modeled survival from time of enrollment. Practical issues that arise and will be discussed include missing covariates, consistency with methods for evaluating other events besides survival, generalizability, and robust subgroup analyses.
