Abstract:

In cancer patients the number of resected lymph nodes have been of interest as a prognostic factor for survival. Total number resected (LN), number of positive lymph nodes (PLN) and lymph node ratios (LNR) have been considered. LNR adjusted for small numbers of resected LN gave more stable results. Optimal cutpoints for the different lymph node variables and ranges of LNR values were derived with different statistical methods. Applied to breast, pancreatic, and colon cancer data these methods identified change points for adjusted lymph node ratios in the range of 0.10-0.20. The two statistical algorithms, resampling method based on the concordance index, and a process based on a re-scaled logrank statistic selected similar change points. The two graphical methods, contours of survival quantiles, and AUC over time, provided a qualitative measure for the choice of lymph node variables.
