Abstract:

In treatment of caner, using predictive biomarkers to identify the right patients to (or not to) treat with certain drugs has become a reality. For example, colorectal cancer patients harboring the K-Ras mutation would not benefit from Cetuximab. When clinicians make a decision weather to treat a patient with certain drugs, one commonly used decision rule is if the expression level of a linear combination of biomarkers (or a single biomarker) is greater than a threshold. Therefore, it is critical to select an optimal threshold. In this talk, we first review methods for selecting the optimal threshold to identify the right patient population. We use a real example in clinical settings with an IHC (immunohistochemistry) biomarker to illustrate issues on selecting the optimal threshold. The impact of measurement errors of the biomarker on selecting the optimal threshold is discussed.
