Abstract:

Lack of a gold standard reference test is a well recognized problem in diagnostic studies. We adjust for this problem in the context of a meta-analysis. We propose a model for joint meta-analysis of sensitivity and specificity of the diagnostic test under evaluation, while taking into account the imperfect sensitivity and specificity of the reference test. This hierarchical model accounts for both within and between study variability. Estimation is carried out using a Bayesian approach, implemented via a Gibbs sampler. The model can be extended to situations where more than one reference test is used in the selected studies. We describe how to extract summary statistics of sensitivity and specificity of the test under evaluation, as well as a summary receiver operating characteristic curve. Methods will be illustrated by application to a meta-analysis of tests for tuberculosis pleuritis.
