Abstract:

Funded by CMS, The Hawaii Demonstration to Maintain Independence and Employment was a randomized controlled trial where treatment group participants met individually for one year with a life coach and a pharmacist to improve their health and employment outcomes. The 190 study participants had a mean age of 48 years; 63% were female, 83% were non-white, and 86% had Type 2 diabetes. Repeated measures ANCOVA was used to evaluate the effect of the intervention. Body mass index, quality of life, and diabetes-related self-efficacy changes were significantly better in the treatment than the control group. Changes in earnings, work productivity, and absences. Findings have implications for other early intervention models intending to improve chronic disease management, prevent disability, and support the employment of individuals with chronic conditions.
