A recently funded study of the impact of oral contraceptive use on the risk of bone fracture employed a randomized recruitment scheme. Previous, related studies have indicated differential participation rates between cases and controls. While data from such schemes may be analyzed within the two-phase framework, ignoring differential participation may lead to biased estimates. To overcome this, we build on the two-phase framework and propose an extension by introducing an extra stage of data collection aimed at addressing differential participation. We propose two general-purpose estimators that correct for both sampling and participation bias. We also propose an additional estimator for the special case where covariates underlying the participation mechanism are discrete. As the fracture study is on-going, we illustrate the methods using infant mortality data from North Carolina.
