Abstract:

The introduction of nephron-sparing and laparoscopic surgeries in the last two decades has modified treatment options for patients with kidney cancer. However, population-based data suggest that the adoption of these techniques has been gradual and concentrated only among select hospitals. Using linked SEER-Medicare data we propose tree-based methods to identify combinations of patient, physician, and hospital variables predictive of partial nephrectomy and laparoscopy receipt, while simultaneously accounting for clustering of patients within a physicians' practice. By clarifying the relative contribution of hospital, physician and patient factors, results from this study can inform efforts towards dismantling the barriers to adoption of these therapeutic options. Such adoption will be an important step toward improving the quality of care provided to patients with kidney cancer.
