Abstract:

Markers for treatment selection can potentially improve patient outcomes and decrease medical costs by targeting treatment to those most likely to benefit and avoiding unnecessary and potentially harmful treatment for others. We argue that the standard approach to evaluating treatment selection markers inadequately characterizes marker performance. We describe a new approach, marker-by-treatment predictiveness curves, which shows how individual marker measurements can be used to inform treatment decisions. Key summary measures include the proportion of subjects whose treatment recommendations change following marker measurement, and the population impact of using the marker to select treatment. The methods are illustrated using candidate markers for selecting women for adjuvant chemotherapy for the treatment of ER-positive breast cancer.
