Title: The Social and Geographic Context of Obesity and Diabetes Epidemics

Abstract:

Background The social and geographic determinants beyond an individual nature for obesity and diabetes remain underexplored. This study examines how social and geographic contexts influence them. Methods We use 2007 BRFSS data and a multilevel structural equation modeling framework to examine the pathways of social and geographic context to this epidemic. Results Obesity and diabetes prevalence and BMI (in 2007) increased from large fringe, large central, medium, small metropolitan, micropolitan, to noncore rural counties. Multilevel analysis shows county-level social and geographic measures explain 65% of state-level and 23% of county-level variations in BMI. Conclusions County-level social-spatial transformations resulting from urbanization processes contribute to significant urban-rural disparities in BMI and may also explain disparities in obesity and diabetes.
