Abstract:

To reduce the risk of transfusion-transmitted malaria (TTM), currently FDA recommends deferral of travelers to malarious regions for 1 year and immigrants from those regions for 3 years. A probabilistic model was developed to evaluate the risks of TTM and benefits (additional donations) of malaria antibody testing of all donations (scenario 1), current deferrable donations (scenario 2), or donations currently deferrable due to travel to Mexico (scenario 3). The model used projected national donor deferrals from a study involving six blood centers and epidemiological reports on malaria cases in the US as inputs. The model considered the size of at-risk donor populations; disease prevalence; asymptomatic probability; donor questionnaire efficiency; sensitivity and specificity of malaria antibody testing. This risk analysis provides a tool to inform risk management of the US blood supply.
