Title: Confidence Intervals for the Ratio of Two Proportions: A Review and Evaluation of Methods

The ratio of two proportions is widely used, for example to estimate relative risk in epidemiology and the likelihood ratio of a diagnostic test. Yet even though this estimate is derived using only four values from a 2 by 2 contingency table, there are at least 12 known methods to estimate confidence intervals. We review these methods, providing heuristic justification for each and a common terminology for easy comparison. We illustrate similarities and differences between methods using data from a study of computerized decision support for pediatric care. We evaluate coverage properties of each method in a variety of scenarios. Confidence intervals and coverage differ substantially between methods when the sample size is small or when either proportion is near 0 or 1. We provide guidance on choice of method for a selection of common scenarios.
