Title: Receiver Operating Characteristic (ROC) Curve for Demographic Variable Dependent Diagnostic Method

Abstract:

Traditional quantitative diagnostic tests often are determined based on a single cutoff. A test is considered "negative" or "positive" for a disease depending when the test result is greater or less than the cutoff. The cutoff value has a great impact on the interpretation of the diagnostic test and consequently patient diagnosis. To improve the clinical sensitivity and/or specificity of the test, one alternative to the traditional approach is to partition the population into several subpopulations based on certain correlated demographic variables and define different cutoff values for each subpopulation (multiple cutoff method). How does this multiple cutoff method impact the clinical sensitivity and specificity? How is the ROC curve impacted by multiple cutoffs? Can ROC analysis be used to determine if the multiple cutoff method provides meaningful benefit relative to the single cutoff method? Can ROC analysis help us determine optimal cutoffs for the multiple cutoff method? We will explore these and similar questions in the context of a quantitative diagnostic test.
