CAHPS ratings and report items have 3-, 4-, or 11-level ordered response categories. To simplify presentations to consumers and other stakeholders, results typically are presented in a dichotomized form. Standard presentations have maximized the power to distinguish plans or other reporting units by splitting near the median. Given the strong negative skewness in the rating of health care, this corresponds to a split distinguishing the top category from all others. On the face of it, there may be important information at the negative end of the scale---a "9" can be very different from a "3" on a 0-10 scale, and the left tail may reflect important adverse events that drive disenrollment and should drive quality improvement efforts. We investigate cuts near the 15th percentile.
