Title: A Comparison of EuroQoL and SF-36 Utilities in End-stage Heart Failure Patients

Abstract:

There are disparities among the preference-based (or utility-based) instruments produced by various methods. Consequently, there is no single preferred preference-based measure of health-related quality of life to estimate quality-adjusted life years (QALYs). SF-6D is a recently developed preference-based measure of health derived from SF-36. It permits a transformation from SF-36 data to preference scores to calculate QALYs. We derive a preference-based single index from the SF-6D using a utility scoring algorithm (Brazier et al. 2002) among end-stage heart failure patients in the REMATCH clinical trial. We compare the resultant preference-based index to the EuroQoL visual analogue scale (VAS) score in order to evaluate the reliability and validity of the SF-6D measures. Our study indicates the two methods do not give comparable results. SF-36 preference score has a much narrower range than the EuroQol preference scores, and it does not have values at the lower end of the health state scale, which suggests a limited use for this instrument in patients with end-stage heart failure.
