Title: A Workflow Scan Statistic

Abstract:

Syndromic surveillance systems increasingly rely on spatial algorithms such as the spatial scan statistic to detect geographic clusters of cases. In many situations, home address is the only spatial information associated with surveillance records. If the exposure occurs in the workplace, reliance on home address may increase the apparent geographic dispersion of cases and reduce the power of detection using the usual spatial scan statistic. We propose a modification of the spatial scan statistic to account for the spatial mobility of individuals following exposure. The spatial scan statistic is modified to make use of a workflow matrix, which is derived from census data and gives the percentage of workers in each geographic subregion (e.g., county, ZIP code) employed in every other geographic subregion. We present results of simulations that show a significant increase in power in various exposure scenarios.
