Abstract:

In diagnostic imaging, blinded read studies are used to compare the performance of modalities with respect to parameters like the accuracy. With more than one observational unit within each patient, i.e. multiple lesions in organs, the patient can be seen as cluster with correlated units. Methods have been developed by Obuchowski (1998), Jung and Ahn (2000), and May and Johnson (1998) that can cope with this correlation stucture. In addition to the adjustment for the correlation within the clusters we have to account for repeated measurements as a result of the paired design where more than one blinded reader determines the images of each patient. This introduces the inter-reader variability, an important source of error in the interpretation of diagnostic modalities, which is estimable in paired designs. For the analysis the reader is treated like a factor in a two-factorial model with factors modality and reader. Similar to the estimation of the treatment effect across centers, this effect can be estimated across readers by averaging the effects by reader with appropriate weights.
