Abstract:

The VA healthcare system stores patient information in an electronic medical record system known as CPRS. CPRS provides access to clinical and appointment information, as well as decision support to clinicians. Clinical reminders prompt providers to conduct annual standardized screenings and follow-up assessments for common disorders. Imbedded within the reminders are self-scoring instruments and evidence-based treatment plans. By entering information in a dialogue box, providers generate standardized clinical notes. A subset of information stored in CPRS is "rolled-up" and stored in a data warehouse. Administrators and policymakers use the data warehouse to 1) identify problems with access, quality and outcomes in specific facilities and service lines, 2) allocate resources in order to address high priority problems, and 3) monitor the impact of resources using CPRS-based performance measures. This presentation will describe how CPRS was used to decide to allocate telepsychiatry resources to rural Community Based Outpatient Clinics (CBOCs) and how the depression clinical reminder is being used to improve the detection and treatment of major depressive disorders in CBOCs.
