Title: Hierarchical Models of Self-reported Symptoms of Poor Glycemic Control and HbA1c in a Diabetic Population

Abstract:

The HbA1c test is a clinical laboratory test that reflects average blood glucose levels over the preceding two to three months. Although patients with higher HbA1c values are more likely to experience symptoms related to hyperglycemia, the relationship between symptoms and HbA1c values has not been well described. We examine the relationship between HbA1c levels and self-reported symptoms possibly related diabetes in the TRIAD study. TRIAD is a national multicenter study of nearly 12,000 diabetes patients enrolled in 10 health plans each having multiple provider groups. The study links patient surveys with their medical record data. We construct separate hierarchical models that predict HBA1c from nine self-reported diabetes symptoms adjusted for patient characteristics known to influence HbA1c levels. We also compare the predictive ability of the separate models with alternative models that use a summary score of symptom severity.
