Abstract:

Hereditary Nonpoliposis Colorectal Cancer (HNPCC) is the most common type of familial colorectal cancer. Previous studies have shown that carriers of germline mutations on the DNA mismatch repair genes MSH2 and MLH1 have an elevated risk and earlier onset of colorectal and other cancers. With a large family-based study carried out at Creighton University with comprehensive information, we derive penetrance estimates for HNPCC mutations in a Bayesian framework. The prior comes from a meta-analysis of previously published estimates. It is then updated with a likelihood computed by MARGENE, a subroutine used by the colon cancer susceptibility model software CRCAPRO. Several statistical problems encountered in the penetrance estimation will be addressed, namely correction for ascertainment bias, adjustment for family-specific effects and extragenetic familial aggregation.
