Sleeve gastrectomy (SG) and Roux-en-Y gastric bypass (RYG) are common bariatric procedures. Bariatric surgery is preferred treatment for morbid obesity but studies comparing 30-day readmissions after these procedures are rare.

Patients from the ACS NSQIP registry, who underwent SG or RYG during 2015 - 2019 were included. The primary outcome was all cause 30-day readmission. Propensity score matching (PSM) was done using demographic variables, and Obesity (BMI > 30 kg/m2). Logistic regression analysis was also accounted for preoperative labs, medical history, major, and minor-postop complications.

Overall 110624 patients underwent SG and RYG. Following PSM a random sample of 60000 cases by proportional allocation was selected. After accounting for other variables the RYG group showed significantly higher odds of 30-day readmission than SG group (AOR: 1.9; 95% CI: 1.7, 2.1).

Even among similar patients, 30-day readmissions were more common after RYG than SG. Given the progressive increase in the proportion of bariatric patients undergoing RYG, hospital programs that aim to decrease readmissions after bariatric surgery need to focus on prevention and management of patients.
