Patient satisfaction surveys are widely used by health plans to evaluate physicians and quality of care. This is a prospective cohort study of adult respondents to the 2010-2015 Medical Expenditure Panel Survey who reported having one or more doctor or clinic visits in the prior year. Patient satisfaction was defined as a standardized composite score of four items pertaining to physician communication with a health care provider (listened carefully, explained things well, showed respect and spent enough time) and overall rating of health care received in the past 12 months. The association of patient satisfaction in the first panel year (year 1) with health care expenditure and utilization in the subsequent panel year (year 2) were assessed. Patients with the highest satisfaction score were matched without replacement to those with lower satisfaction using propensity scores estimated from sociodemographic characteristics, insurance coverage, health status, and year 1 total health care expenditure and utilization. Balance in covariates was assessed before and after matching using p-values and standardized differences. Sensitivity analysis of unobserved confounding was conducted.
