Abstract:

Causal inference methods for the evaluation of packages of prevention interventions for HIV are needed because many interventions only offer partial protection. We propose an approach to evaluate the causal effects of package components in a multifaceted intervention. Only some participants randomized to the intervention are exposed directly. The individual effect is that among directly exposed participants beyond being in an intervention network; the disseminated effect is that among participants engaged with those directly exposed. We estimated package component effects in a Phase III network-randomized HIV prevention trial among persons who inject drugs and their risk networks. We used marginal structural models to adjust for time-varying confounding. The index participant in an intervention network received an initial peer education intervention and boosters and all participants were followed to ascertain risk behaviors. We evaluated the individual, disseminated, composite, and overall effects of the initial intervention and boosters. These methods will be useful for evaluation of the causal effects of package interventions in a single study with network features.
