Simultaneous administration of all age-appropriate doses of vaccines is the standard practice for childhood vaccinations. Vaccination coverage for the fourth dose of pneumococcal conjugate vaccine (PCV) among children 19-35 months has not reached the Healthy People 2020 target of 90%. Research has not been conducted on the factors associated with missed opportunities for simultaneous administration of the fourth dose of PCV. A missed opportunity is defined as failure to administer an age eligible fourth dose of PCV during the same provider visit. This study analyzed 2008-2015 National Immunization Survey data to assess the risk factors for missed opportunities. The results shown that 9.2% (8.9%, 9.5%) of children missed the opportunity in the U.S.; across all 14 risk factors considered the missed opportunities ranged from 4.1% to 24.0%;the timeliness of the 1st through the 3rd dose of PCV were the significant and modifiable risk factors both in binary and multivariate analyses; the adjusted risk ratios for late vs. on-time are 1.5, 1.8, and 3.0. Implementing the strategy for improving on-time vaccination of the first 3 doses of PCV could reduce missed opportunities significantly.
