While simulation of a clinical trial is valuable even for simple designs, it becomes indispensable as the goals of the design become more ambitious. One such goal is enrichment, in which the design attempts to identify a population that benefits from a drug or device. Simulation can help test rules for stopping enrollment of patients in subsets of the population, the statistical model used for the dependence of outcomes on patient characteristics, and the effect of enrichment on operating characteristics. We illustrate the approach using an example from stroke treatment.
