Many complementary and alternative medicine (CAM) therapies involve a multi-modal, individualized, holistic approach to health care which can be difficult to study in the standard randomized, blinded, controlled trial. This paper will focus on the challenges specific to body-based CAM patient-oriented research such as spinal manipulation or massage therapy. Complex issues in designing effective clinical trials of these types of therapies include: standardization of treatment delivery; defining and ensuring delivery of therapeutic doses; the inability to blind the provider to treatment group; and choosing appropriate control groups. For example, dose in body-based therapies is often defined as frequency of care. However, the force, contact, and number of manipulations per treatment visit also constitute aspects of dose for spinal manipulation. Current investigations of and strategies for dealing with these issues in trials of spinal manipulation at a chiropractic research center will also be discussed.
