Abstract:

We aimed to study the excess health care expenditures for persons with pneumococcal disease. The dataset contains health care costs and the occurrence time for patients diagnosed with the disease and for a matched control. First, we analyze the age-specific per-patient cumulative costs over time, using GEE. The mean structure is described by fractional polynomials. The QIC criterion is used to compare different mean and correlation structures. For all age groups, health care costs for diagnosed patients are significantly larger than those for undiagnosed persons at the time of diagnosis and long before and after the diagnosis. In a second analysis, joint modeling of costs and the time-to-event is performed using mixed models. The time-to-event is assumed to follow an exponential distribution, while the costs themselves are modeled using a Gamma distribution, conditional on the time.
