The Prostate Cancer Prevention Trial prostate cancer risk calculator was developed to aid urologists in screening healthy men for referral to biopsy based on prostate-specific antigen (PSA) and other clinical risk factors. This paper sought to use appropriate statistical methods to clarify the relationship between body mass index (BMI), PSA and prostate cancer risk, and elucidate the role of BMI in current screening practices using 3697 men in the San Antonio Biomarkers Of Risk study. Our study showed that BMI was not a risk factor for prostate cancer. Among non-cancers the mean PSA monotonically decreased with increasing BMI. Adjustment for diminished levels of PSA in overweight and obese men did not increase the accuracy of PSA-based risk assessment. However, due to lower levels of PSA, overweight/obese men may suffer diminished diagnostic opportunities in PSA-based screening programs.
