Quantitative scientists play vital roles in pharmaceutical medicine, and need to become an integral part of decision making processes. I will share some of my observations about challenging issues we face, focusing on clinical trials. Examples: (1) In a non-inferiority study, there are many difficulties involved in setting an NI margin. More quantitative information regarding cost-risk-benefit is needed here. (2) In monitoring a study, we can utilize prediction ideas to help in decision making. (3) In the analysis of efficacy and safety data, it is important to come up with quantitative ways to assess risk-cost- benefit at a personal level and then aggregate the information for an overall assessment of new treatment strategies. I will discuss these issues in a more general setting with real examples.
