Abstract:

Hospital report cards are being published with increasing frequency. One objective of hospital report cards is to identify hospitals that require quality improvement initiatives. A second objective is to identify hospitals from which one does or does not want to purchase health care. These objectives imply that a decision-theoretic framework can be adopted in designing hospital report cards. In producing hospital report cards two potential errors can result: first, false positives in which hospitals that truly provide acceptable quality of care are incorrectly classified as providing unacceptable quality of care. Second, false negatives in which hospitals that truly provide unacceptable quality of care are classified as providing acceptable quality of care. False positives can result in costs to hospitals in terms of decreased revenue, diminished staff morale, and more.
