More than 30 years into the HIV epidemic, effective and scalable interventions remain elusive, particularly in areas with high levels of infection. The SEARCH study is designed to assess the impact of a test and treat strategy at the community level to reduce levels of HIV infection. It is currently being implemented in 32 communities in Kenya and Uganda. Statistical issues center on how best to measure cumulative HIV incidence at the community level over a 5 year period given loss to follow-up and migration, and subsequently how to best compare communities offered early anti-retroviral therapy as compared to using employing current in-country guidelines. Comparisons with related work on community intervention trials to assess reduction in incidence of Dengue fever will also be discussed.
